তথ্য অধিকার আইন ২০০৯ এ তথ্য প্রাপ্তীর জন্য নির্দিষ্ট আবেদন ফরম রয়েছে। তথ্য অধিকার (তথ্য প্রাপ্তী সংক্রান্ত) বিধিমালার বিধি-৩ এ দ্রষ্টব্য। এছাড়া একই আইনে অভিযোগ ও আপীল ফরম রয়েছে যার মাধ্যমে তথ্য প্রাপ্তীর ক্ষেত্রে অভিযোগ বা আপীল করা যাবে।


The Right to Information Act 2009 contains specific application forms for obtaining information. Note Rule 3 of the Right to Information (Receipt of Information) Rules. The same law also contains complaint and appeal forms through which complaints or appeals can be made in case of obtaining information.
